Your Records

All requests for medical records must be made in writing by completing an Authorization for Release of Medical Information form (see below). Only “hard” copies or faxed copies of this form can be accepted. At present, we are unable to accept e-mail requests for medical information.

Since the Director of Health Services must review all information before it is released, requests for records may take up to one week to process.
